To provide the requestor with a Certificate of Liability Insurance from Las Positas College, please fill out the and contact the .
The District Business Services Office will need to know:
- The date and time of the event;
- Who is sponsoring it and the participants;
- The address of the certificate holder;
- If they need to be additionally insured. If so, they will need a contract for use of facilities.
