Admissions Forms (PDFs)

  1. Please download the PDF form to your computer and then open it with before filling out.  If you complete it in the web browser, no information is saved.
  2. Use for your email subject header: Your Full Name--Name of Petition 
  3. It is strongly recommended that students submit admissions forms via email () to the email address listed on the form.

Admissions Forms (InformedK12)

  1. About InformedK12: The forms below are processed using the InformedK12 online software that helps facilitate the completion and routing of forms. These forms will be automatically routed to the person you indicate on the submission screen. All 91ÖØ¿Ú employees and students can fill out and submit forms electronically for processing (without an account). The software comes with a myriad of benefits, including faster processing times, more transparency on the status of submissions, unlimited online storage, and more. All forms being facilitated through InformedK12 must be submitted via InformedK12; PDF versions will not be accepted. 
  2. How to Submit a Form: Please note that you do not need an account to submit a form.

    2. Enter your full name and (no personal emails)
    3. Fill out the form (view the InformedK12 Instructions for guidance)
    4. Select the appropriate person for routing
    5. Once saved and/or submitted, you will receive a link via email to view the submission and its status
